Set per-outlet item visibility override

Set a per-outlet visibility override for an item in a section. When this override exists, it takes precedence over the draft-level default visibility during publish. This allows hiding/showing items on specific outlets. **Publish behavior:** When publishing to an outlet, if an override exists for an item+outlet, the override's `visible` value is used. Otherwise the draft default is used.
